"What is Life is Very Good​?"

     Life is Very Good is a series of two events; the Evening of Prayer (January 23) and the Morning Rally and Mass (January 24).  St. Luke Youth Ministry will be taking a group of teens to both events!  The Evening of Prayer is be an opportunity to celebrate all life and to pray for the success of the March for Life in changing our hearts and those of others in this country.  The Morning Rally prepares us to take to the streets of Washington D.C. to make our voices heard as we proclaim the gospel of life.  For information on the speakers & musical acts that will be featured during the Evening of Prayer and Morning Rally and Mass, please check out the Diocese of Arlington website here!  Following the Morning Rally and Mass we will travel to D.C. to participate in the March for Life.

     For the Evening of Prayer (January 23) we will depart St. Luke at 6:00pm and return at 10:30pm.  The event will be held at Eagle Bank Arena on George Mason University’s Campus.  The cost of the Evening of Prayer is includes the keynote, concert, and transportation; see the Permission Slip for details.

     For the Morning Rally and Mass (January 24) and the March for Life we will depart from St. Luke at 8:00am and return at 5:00pm.  We will travel together from the parish to EagleBank Arena (8:00-11:30am), then to the March for Life (11:30am-4:00pm), and then back to the parish arriving by 5:00pm. The cost of the Morning Rally and Mass and the March for Life includes transportation, Rally & Mass, Chick-Fil-A lunch, and the March.  The weather will likely be very cold, so be prepared to dress warm!  Transportation to the March for Life will be provided following the Morning Rally & Mass.

​Parent Permission Agreement
Parent Permission Agreement:  As parent/legal guardian of the youth named on this form, I give my permission to participate fully in above program from the start date above to the end date above.
 
I agree to indemnify and hereby release the Most Reverend Michael F. Burbidge Bishop of the Catholic Diocese of Arlington and his successors in office, as well as the Catholic Diocese of Arlington and all Diocesan clergy, employees, volunteers, and participating parishes and schools from any and all liability, claims, demands for personal injury, sickness and death, as well as property damage and expenses of any nature whatsoever which may be incurred by the undersigned of the participant resulting from said participant’s involvement in the above mentioned event (including transportation to and from the event). Furthermore, I on behalf of the participant hereby assume all risk of personal injury, sickness, death, damage, and expenses resulting from said participant’s involvement in the above described event.
 
Informed Consent to Medical Treatment: I request that in my absence the above-named minor be admitted to any hospital or medical facility for diagnosis and treatment. I request and authorize physicians, dentists, and staff, duly licensed as Doctors of Medicine or Doctors of Dentistry or other such licensed technicians or nurses, to perform any diagnostic procedures, treatment procedures, operative procedures and x-ray treatment of the above minor. I have not been given a guarantee as to the results of examination or treatment. I authorize the hospital or medical facility to dispose of any specimen or tissue taken from the above-named minor. I assume full responsibility for all costs of such treatment. Further, should it be necessary for the participant to return home due to medical, disciplinary, or other reasons, I do hereby assume responsibility for the participant’s transportation home and any costs related thereto.
 
Photo, Press, Audio, and Electronic Media Release: I authorize the Catholic Diocese of Arlington, its parishes, its schools and/or the Arlington Catholic Herald to use and publish my child’s photograph, video and/or audio recording along with their name identifying them for educational, news stories, illustration and/or marketing purposes.
 
Use of electronic communication: I authorize St. Luke’s Youth Ministry and its Volunteers to communicate with the participant through the following electronic means: phone call, email, text messaging, Facebook, and Instagram. This permission is granted with the understanding that communication will take place at reasonable times and in limited durations. Parents/guardians who do not wish to grant this permission should notify St. Luke’s Office of Youth Ministry in writing.
 
I understand and hereby agree to the terms and conditions of the participant’s involvement in the above described event and I freely execute this Acknowledgement with full knowledge of its content.
Online Payment
     To pay for Ultimate Outing there are 2 options; please either follow the link below to participate in online payment through Faith Direct or drop-off a check to our office for the registration total.  If you wish to do online payment, simply follow the link below, fill out the Amount section with the total cost of all participants you wish to register, and specify in the Note section what the payment is for!  If you are registering multiple people, please fill out a registration form for each, but you can add-up the total payment cost for each and provide a single payment on Faith Direct of the total.  Just be sure to indicate in the Note section who's registrations you are paying for!  Email us if you have any questions about payment at youthministry@saintlukemclean.org.
