 Saint Luke Parish School of Religion (PSR) is open to all
 REGISTERED parishioners.

 Email is our primary method of communication with parents. In case of an emergency
 every attempt will be made to contact the parents first. We will then attempt to contact
 the Emergency Contact (someone other than a parent) if we are not able to reach a
 parent.

 Completed Enrollment Forms are processed on a first-received basis. We encourage
 you to return completed forms with the proper fees as soon as possible.

 I understand enrollment in and attendance at Religious Education class DOES NOT
 excuse my child's need to attend weekly Mass. (The way you as a family approach the
 Catholic faith teaches your child more than he or she will ever learn in weekly classes.
 Attending Religious Education only and not Mass also sends your child a confusing
 message.)

  Fee Schedule*
  Pre-K through 12th grade
  8th grade: each student $225**
  **includes Sacramental fee
  One child $125
  Two children $225
  Three children $325
  Each add'l child +$50
  Enroll before June 30 to receive a discount of $25.
  After August 1, 2017, there is a Late Fee of $50 per family.
  *Tuition assistance is available. Please contact the Faith Formation Office at 703-356-8419. No one is refused enrollment for inability to pay.
  * A copy of each child's Baptism certificate must accompany the Enrollment Form or be on file.
  If your child's Church of Baptism is not listed in the drop down box, select "Other" and type the name of the church in the BAPTISM box.
  Please let us know in the Student MISC. INFORMATION box:
  - Did this child attend a Faith Formation Program or Catholic school in 2016-2017? If yes, where?
  - Please note if this child is in 3rd grade or above and has not received Baptism,
  First Penance and/or First Communion.
  - In order to ensure your child's well-being, please list any allergies or medical conditions and/or special need situations of which you wish us to be aware.

  - Please indicate if this child would like to join the Saint Luke Children's Choir: Choristers (grades 2 to 7) on Thursdays from 4:30 to 5:15 pm.
