Sponsor FormsIf you have been asked to be a godparent or Confirmation sponsor, you will need to submit a Sponsor Form or Certificate of Eligibility to the church where the sacrament will take place. You can find the forms here and print them out. You will need to complete the top portion and then come to the parish office for the pastor to witness and seal the document.
