Sponsor Forms
​for Baptisms and Confirmation

If you have been asked to be a godparent or Confirmation sponsor for a sacrament taking place at another church, you will need to send that parish a Certificate of Eligibility or Sponsor Form.
Please choose the appropriate form below. You will need to complete the top portion of the form and then bring it to the parish office to have the Pastor witness and sign the form. Godparents and Sponsors must be active members of the parish and have received the Sacraments.
